GreenStar Certification Questionnaire


The Florida Wildlife Federation would like to acknowledge its members’ personal commitment to a clean and healthy environment through the Florida GreenStar Certification program. Please answer the following questions by entering the number under “Point Value” that best describes the frequency that you engage in each beneficial activity. ( 5 = All of the time; 3 = Some of the time; 0 = Never)

If you receive a score of 120 or higher, we will send you a Florida GreenStar Certificate to honor your efforts. Thank you for taking part in this new program.

*If you live in a condominium or other residence with no yard, give yourself 3 points on the * questions below. Passing grade for receiving an FWF GreenStar Certificate is 120.

ENVIRONMENTALLY BENEFICIAL PRACTICES

Outdoor Impact

I limit the use of fertilizers, pesticides and herbicides in my yard.
